PROJECT 2地域医療再構築プロジェクトProject for the Reconstruction of Community Health CareTohoku University Reconstruction Action東北大学復興アクション被災地の臨床研修病院を対象とした出張スキルスラボ気仙沼市立病院は宮城県で東北大学から最も遠い、被災地にある臨床研修病院です。2013年から年に1回、東北大学クリニカル・スキルスラボのシミュレーターを気仙沼市立病院に持ち込み、教員が研修医を対象にシミュレーション教育の機会を提供しています。Outreach program of our clinical skillslaboratory for a postgraduate teachinghospital in a devastated areaKesennuma Municipal Hospital,located in the area devastated by theGreat East Japan Earthquake, is thefarthest postgraduate teachinghospital in Miyagi prefecture from theclinical skills laboratory of TohokuUniversity. Our faculty members havetaken several simulators to thishospital once a year since 2013 andprovided the residents and fellowswith an opportunity to experiencesimulation education.緊急気道管理トレーニングコースの開催Hands-on training for airwayemergencyAn airway emergency could leadto death if an appropriateintervention cannot be performedimmediately. Therefore, to masterairway management skills iscrucial. This seminar provides localhealthcare providers advancedairway management skills withreal clinical devices.全国の医学生を対象とした被災地医療体験実習総合地域医療研修センターでは、全国の医学生を対象に被災地医療体験実習を実施しています。2011年から2014年までに本プロジェクトの支援を受けた全国からの医学生102名が東日本大震災の被災地を訪れました。被災地の医療がどんな影響を受け、今何が必要かを学び、地域医療・災害医療を担う人材の育成を行っています。Program for medical studentsfrom all over Japan to visitdevastated areasFrom 2011 to 2014, with supportfrom our project, 102 medicalstudents from all over Japanvisited the devastated areas hitby the Great Earthquake. Theylearned how health care systemshad been affected by thedisaster and what was neededthere.訪問看護師を対象とした看護技術研修会緊急気道確保方法について、地域の医療従事者を対象に実際の気道確保器具を用いた実技トレーニングを行い、スキルの向上を図っています。宮城県訪問看護ステーション連絡協議会と共催し、訪問看護師を対象とした、CVポート管理、呼吸音の聴診、ストーマケア等の看護技術研修会を開催しています。Nursing skills workshops forhome-visiting nurseIn collaboration with the liaison councilof home-visit nursing stations in MiyagiPrefecture, we hold workshops onnursing skills, such as management ofCV catheters, stoma care, and chestauscultation for home-visiting nurses.バーチャル顕微鏡と遠隔会議システムを用いた臨床病理検討会臨床医と病理医が情報を共有することは、適切な治療選択に極めて重要です。被災地における病理医不足のため、バーチャル顕微鏡を用いた遠隔会議システムを使用しています。Clinicopathological conferencesusing virtual microscopies and anonline meeting deviceTo share the clinical and pathologicalinformation of patients betweenpathologists and clinicians is pivotal for theselection of accurate therapy and forunderstanding the diseases. The virtualmicroscope and the online meeting deviceenable us to view microscopic images andto discuss each case in remote locations.大規模災害に備えた災害口腔科学専門研修会の実施東日本大震災は、歯科における様々な課題を浮き彫りとしました。歯学研究科では、今回の震災を教訓とし、大規模災害の歯科的対応を検証、再考する研修会を開催しています。Seminars of disaster oral science preparingfor future large-scale disastersThis seminar aims to develop well-organizedsystems in dentistry to respond to large-scaledisasters. At the Great East Japan Earthquake, wefaced various problems due to insufficientarrangements of cooperation with otherdisciplines and the lack of information. The dentalcommunity is required to establish a cooperativesystem composed of universities, the governmentand dental associations and to share commonknowledge for large-scale disasters in the future.被災地における災害歯科医療学実地研修会の実施本研修会は、被災地に赴き、震災時に医療現場で対応にあたった歯科医師や被災された方々との対話を通じ、大規模災害時に医療が果たすべき役割を検証することで、災害時に対応しうる人材の育成を目的にしています。本研修会の実施を通じ、言葉では表現できない大切な何かを感じ得ることを期待しています。災害歯科学の世界への発信東日本大震災で得られた教訓、課題を後世に伝えることは、震災を経験した唯一の総合大学である東北大学の使命であると考え、同じように大震災を経験した四川大学とのシンポジウムの開催を始め、海外への情報発信行っています。また東北大学発のデンタルチャートの国際規格化に努めています。SimNight・SimMarathonの開催緊急患者の対応について、地域の医療従事者を対象に平日夕方・休日を利用し、高機能シミュレーターなどを用いて、現場と同等のリアリティでチーム医療を実践します。Authentic simulations:SimNight and SimMarathonIn order to deal with critically ill patients, it isrequired that healthcare providers have theability to diagnose and perform properinterventions immediately. This seminarprovides an environment identical to the realworld, so the participants can learn teammanagement skills with authentic simulatorsand devices.On site seminars regardingdisaster dental care atsuffering areasThis seminar aims to cultivatedentists and co-dental staff whocan respond appropriately whendisasters occur. The participantsare expected to be conscious ofroles of dentistry in majordisasters throughcommunication with thedisaster sufferers and thedentists who were engaged inthe dental forensic identificationand dental care. We also hopeto inspire the participants'consciousness and motivationfor disaster medicine.Transmission of "disaster oralsciences" to the worldWe have a responsibility to conveythe knowledge of disaster oralsciences including dental forensicidentification, dental care forsuffering people, and requiredsystems, which we are establishingfrom the experiences in the disaster.We have held several internationaland domestic symposia. We alsodevote ourselves to establish aninternational standard dental chartfor victim identification.歯学部学生への災害歯科学講義の開講歴史的・世界的大災害を経験した唯一の総合大学である東北大学で学ぶ学生として、早い段階で災害歯科医療に対する高い意識付けを図っています。Lectures titled "Disaster dentalmedicine" for undergraduatestudents at School of DentistryThe School of Dentistry offerslectures that develop awareness andcomprehension useful in disasterdental medicine for theundergraduate students includingforeign students in Tohoku Universitythat had valuable experiences in theGreat East Japan Earthquake. Wehope to develop young dentists whocan play important roles inlarge-scale disasters in the future.13 14
